Job Description:

LTC Physician Care, PC provides Primary Care (PCP) services to Skilled Nursing and Long-Term Care facilities across Michigan, ensuring their unique needs are met with expertise and compassion. LTC is seeking a dedicated Nurse Practitioner to join our team as Primary-Care Providers. for facilities located in Lamont, Forest Hills and Grandville, MI

Requirements and Qualifications

• Experience in Family Medicine, Internal Medicine, or Geriatrics preferred.
  • We are happy to consider new graduates.
    • Candidates must hold a valid and unrestricted NP medical license in Michigan.
    • Candidates must hold a valid DEA license in the state of Michigan.
    • Candidates must have completed a valid U.S. certification program Job Structure
      • This is an in-person position
      • Providers will commute directly between home/facilities
      • Nurse Practitioners work in coordination with an LTC Collaborating Physician
      • Nurse Practitioner is responsible for the day-to-day care of their patients
      • After-Hours on-call is shared among all LTC Providers Compensation and Benefits
        • Productivity Based Compensation Model (No Cap)
        • $10k SIGN ON BONUS
        • 15 PTO days annually
        • $2,500 Annual CME allowance plus 3 PTO days to use for CME attendance
        • Company paid subscription to Up To Date , both mobile and laptop
        • Monthly phone stipend
        • All medical licensure expenses reimbursed by LTC
